Because students sometimes make errors on the FAFSA, the U.S. Department of Education (USDE) has established a process for verifying the information submitted on the FAFSA and empowering colleges and universities to make corrections on a FAFSA. This process is called Verification. Verification is the USDE's means of testing the accuracy of the information submitted on FAFSAs and by doing so ensuring the integrity of the student financial aid programs. Schools are authorized to collect certain documentation, for review, in order to complete this process. If the information submitted conflicts with the information provided on a FAFSA schools are authorized, and obligated, to make a correction to the FAFSA before financial aid eligibility can be accurately determined and funds awarded.

If you are an Independent student selected for Verification for the 2012-2013 academic year (which encompasses the fall 2012, spring 2013 and summer 2013 semesters), and you have reviewed your “Unsatisfied Requirements” on CSU X-Press, please download this form, complete it and submit to our office as soon as you are notified that you have been selected for Verification. If you did not choose to use the IRS DATA RETRIEVAL TOOL you must submit a 2011 IRS TAX RETURN TRANSCRIPT along with this Worksheet.
PLEASE NOTE: Due to changes in USDE procedures for the 2012-2013 academic year, we will no longer be able to accept copies of U.S. 1040s as part of the documentation to complete the Verification process. To obtain an IRS tax return transcript, go to www.IRS.gov and click on the “Order a Return or Account Transcript” link, or call 1-800-908-9946. Make sure to request the “IRS tax return transcript” and not the “IRS tax account transcript.”
If you are selected for Verification, but do not complete the process, no financial aid funds, including loans, will be credited to your student account.
