Step By Step Instructions

  1. Click here if you have not already completed the New Student Accommodations Request form
    • Once form is completed, check the acknowledgment box and select "submit request"
  2. Email a copy of your student detail schedule to the Abilities Office
  3. Submit your Verification of Disability Form and any relevant medical documentation (see documentation guidelines) to the Abilities Office via email or by scheduling an on-campus appointment.
  4. Once steps 1-3 are completed, the Assistant Director of Abilities will schedule and complete an intake interview with the student.
  5. After the intake interview, the student reads, initials and signs the Student Responsibilities Attestation document and emails it to the Abilities Office

    • Once form is completed, check the acknowledgment box and select "submit request"
  2. Email a copy of your student detail schedule to the Abilities Office
  3. Reads initial and sign the Student Responsibilities Attestation document and email it to the Abilities Office

 

Final Steps by All Students

  1. The Abilities Office completes file review and sends the student the Accommodation Agreement (please allow 2-4 business days for processing).  **The Accommodation Agreement IS NOT the Accommodation Letter and DOES NOT go to the professor.
  2. If the student agrees with the accommodations outlined in the agreement, the student returns the signed Accommodation Agreement via email to Abilities Office at abilities@csu.edu (or contacts the Abilities Office for corrections)
  3. The official Accommodation Letter is emailed to the STUDENT within 1-2 business days
  4. The STUDENT emails the Accommodation Letter to the professors (following instructions found in the email or here)

 

Final Steps by Abilities Office

The Abilities Office communicates a student's approved accommodation via letters that are requested and delivered by the student to their instructors.  The letters include all of the classroom accommodations that are approved for an individual student.
