All registered undergraduate students taking 12 or more credit hours, graduate students taking 9 or more credit hours, pharmacy students taking 12 or more credit hours, and doctoral students taking 6 or more credit hours are automatically enrolled into the mandatory sickness and injury insurance through the university each semester. All International students are required to purchase the insurance plan for the school year.  Part-time students taking 6-11 credit hours are eligible to enroll in this plan.

The mandatory requirement applies to students who enroll full-time in on-campus courses only, or a combination of off-campus (Extension) and on-campus courses.  Note that students enrolled in all web/online courses are not charged for the insurance plan.

Full-time students must complete and submit an online waiver or enrollment form each Fall and Spring semester that they are enrolled.

If you have insurance: Students who are adequately insured have the option of waiving out of the mandatory health plan and having the charge removed from their tuition bills. Failure to submit an online waiver or enrollment form by the deadline will obligate students to pay the Full-time Student Insurance Premium coverage indicated on your tuition bill.  If your waiver is denied, and you would like to be reconsidered, you may provide proof of coverage to the Office of Student Health Insurance.

 If you are not insured: Full-time students who do not have insurance must purchase the mandatory insurance provided through the university.

Adequate insurance must be provided through an acceptable health insurance underwriter such as Aetna, Blue Cross/Blue Shield, United Healthcare, Cigna, to name a few,  and must be at least comparable in coverage to the plan provided through CSU.  Click Here to review the plan offered by the university under United Health Care Student Resource. 

Medicaid insurance plans are accepted. They offer a 24/7 consultation and coverage without any out-of-area limitations. 

If you want to purchase insurance: Students who wish to purchase the mandatory Student Injury and Sickness Insurance Plan provided through the university must complete and submit an online enrollment form to activate coverage as soon as possible. 

To settle disputes:  You will receive a confirmation email at your CSU email account that your waiver or enrollment form was accepted or denied.  A copy of your confirmation email will be required for any waiver/enrollment disputes.  NOTE: CSU reserves the right to reverse waivers that were originally approved.

Before a student can go online to waive out of or enroll into the mandatory insurance plan, he/she must be enrolled full-time for at least 48-72 hours.  This window of time allows the Office of Student Health Insurance to retrieve your registration status and upload it into the United Healthcare website.

As students names are uploaded into the insurance website, they will receive notification through their CSU email account that they are then able to log-in to the website to complete a waiver/enrollment form.

The waiver/enrollment period for Fall 2024 Semester regular 16-week session is as follows:

  • Begins: August 1, 2024
  • Ends: September 20, 2024 at Midnight

The plan provided through Chicago State University is underwritten by UnitedHealthcare Insurance Company and is based on policy 2022-2363-1 for Spring 2022 Semester.  The plan brochure provides details of coverage including costs, benefits, exclusions, any reductions or limitations and the terms under which coverage may remain in force.  Click here to access a downloadable copy of the brochure.

Part-time students who enroll in 6-11 credit hours of coursework are allowed to voluntarily enroll with United Health Care into the student health insurance plan provided through the university. 
Eligible students who do enroll may also insure their Dependents. Eligible Dependents are the student’s legal spouse, partner by civil union, Domestic Partner, and dependent children under 26 years of age. Dependent eligibility expires concurrently with the Insured student.
Students are NOT automatically charged for the insurance plan during the summer sessions. Those who have purchased the insurance plan for the Spring semester get a bonus and are covered for the summer, as well. However, anyone who needs health insurance during the summer and enrolls in at least 6 credit hours for the summer may purchase the insurance.  Contact the Office of Student Health Insurance for more information.

  • Failure to submit an online waiver or enrollment form by the deadline will obligate students to pay the full-time student insurance coverage and accept coverage.
  • You must complete and submit a waiver/enrollment form each Fall and Spring semester that you are enrolled full time. 
  • You must register into your classes at least 48-72 hours before you are able to go online to waive out of or enroll into the plan.
  • To buy insurance, you must submit an enrollment form to activate coverage as soon as possible.
  • If your waiver is denied, you may provide proof of coverage to the Office of Student Insurance for reconsideration.
  • A confirmation email will be required for any waiver or enrollment dispute.
  • All International students are required to purchase this insurance plan for the school year. 
  • Part-time students taking 6-11 credit hours may participate in the plan on a voluntary basis.
  • You can download your insurance identification card from the UHCSR website.
