Checklist

• Review the S.U.R.S website at  to determine retirement eligibility and begin planning process
• Contact CMS at 800-442-1300 to confirm the process for medical benefits regarding the state
• Request and complete the S.U.R.S Retirement application and Tax packet
• Reach out to Medicare to start the process for your health benefits by calling 1-800-772-1213 or visiting Social Security website
• Request and complete an application for other reciprocal Illinois public retirement systems, if applicable
• Contact Human Resources/Benefits office within 30 days of retirement to complete required notification at HR@CSU.edu.
