To secure the University Certification of Education IDPR documents 

1. Using Adobe Sign, complete the (IDFPR) ED form. No further acftion is required. If approved you will receive CSU's signature and seal on the document you submitted.
2. Visit the  Website for more information or need more detailed instructions.

The form will be mailed to the address you submitted.

If you prefer to pick-up your form, email your request to: mswverify@csu.edu,

• Provide your name, phone number and email address. You will be notified when the forms are ready. The forms will be available for pick-up from the Office of the Registrar.
